Go down to your local law school and have the Prof use it as a teaching tool and

have the students draw one up for ya ! 

Better than shopping around on-line, and more specific to you and your State.

Call the local Law School, ask the Prof what he/she suggests you look for on-line.

You may also call your State's Bar Association, easy to find on-line, and have them direct you!
