JAKARTA: Indonesia has identified its first case of the Omicron coronavirus variant, the country’s health minister Budi Gunadi Sadikin said on Thursday.

The variant was detected on Wednesday evening in an employee at the Wisma Atlet hospital in Jakarta, who had no history of overseas travel.

The health minister said so far there was no community transmission, but there were five more suspected omicron cases, including two Indonesians who had recently returned from the United States and from Britain, and three Chinese nationals currently in quarantine in Manado, North Sulawesi.

The government is waiting for genomic sequencing to determine those cases.

The omicron variant, first detected in South Africa and Hong Kong last month, has now been reported by more than 70 countries, according to the World Health Organization (WHO), including in neighbouring Malaysia, Singapore, Thailand and the Philippines.

Preliminary evidence indicates that Covid-19 vaccines may be less effective against infection and transmission linked to the variant, which also carries a higher risk of reinfection, the WHO said.

Indonesia Reports First Omicron Case in Jakarta Health Facility

Arys Aditya 09:29 AM IST, 16 Dec 2021 11:20 AM IST, 16 Dec 2021

(Bloomberg) -- Indonesia has confirmed its first case of omicron variant of the coronavirus and detected five other probable cases in the capital Jakarta and the eastern city of Manado. A cleaning worker at Wisma Atlet in Jakarta was found to have the omicron variant when tested on Dec. 15, Health Minister Budi Gunadi Sadikin said in a Thursday briefing. Whole genome sequencing is being carried sequencing is being carried out on two other cases found in the same facility that’s dedicated for coronavirus handling, with the result expected in three days. There’s no sign yet of a community transmission, Sadikin added. “We urge everyone to remain calm and not panic with the detection of the first omicron case,” he said. “We also call on people to refrain from overseas travel unless  necessary.”

Southeast Asia Races to Vaccinate as Omicron Threat Looms: Chart Indonesia has lengthened its required quarantine period for foreign arrivals to 10 days, from as short as three days, to curb the spread of the omicron variant. The government remains wary of a potential virus resurgence following increased travel during the year-end period, even as it pared back planned restrictions due to improved testing and vaccination.

The confirmed omicron case has no history of overseas travel and shows no symptoms, the health minister said. The probable cases include two Indonesian citizens returning from the U.S. and the U.K. who are now quarantining in Jakarta and three Chinese citizens entering through Manado.

The government has stepped up its whole genome sequencing activity to 10% of confirmed cases, from 5% previously. It will also use more polymerase chain reaction tests with S-gene target failure to be able to detect the omicron variant faster, Sadikin said.

Indonesia Reports First Omicron Case

BY :MARIA FATIMA BONA

DECEMBER 16, 2021

Jakarta. Indonesia on Thursday reported the first patient who tested positive for the Omicron variant.

The patient, identified by initial N, is a cleaning service worker at the athlete’s village in Jakarta that has been converted into the country’s biggest isolation facility since the wake of the Covid-19 outbreak in mid-2020, Health Minister Budi Gunadi Sadikin said.

“We sent data to the GISAID and it returned with confirmation that it was Omicron based on the genome sequencing,” Budi said, referring to the coronavirus global data-sharing organization.

He said three athletes’ village workers tested positive for Covid-19 on December 8 and the samples were sent to the Health Ministry’s research center for genome sequencing two days later.

“Of the three cases, only one is detected as Omicron,” Budi said.

The three patients were asymptomatic and the latest PCR tests indicated that they already recovered, he said.

Omicron Arrives in Indonesia

BY :JAKARTA GLOBE

DECEMBER 16, 2021

Jakarta. The government has announced on Thursday that the Omicron variant of the Covid-19 virus had been  detected in Indonesia, being the latest country to detect the most transmissible Covid-19 variant.

Health Minister Budi Gunadi Sadikin said that one of the three cleaning workers at Wisma Atlet Kemayoran, a government-sponsored quarantine and isolation facility for Covid-19 patients and overseas travelers in Jakarta, tested positive for Omicron following gene sequencing tests.

"Last night, Dec 15, 2015, the Ministry of Health found out that patient N was a confirmed Omicron variant patient," Budi said in a press conference on Thursday, identifying the patient by their initial.

Patient N is a cleaner at Wisma Atlet, Budi said. The patient and his coworkers took a test on Dec 8. Later the Ministry of Health had their samples for genome sequencing.

"We received samples on Dec 10, there were three cleaning workers who were positive, but one was positive for omicron," Budi said.

The World Health Organization (WHO) said Omnicron was spreading at an unprecedented rate worldwide, reaching 77 countries since first detected in South Africa in November. WHO Director-General Tedros Adhanom Ghebreyesus said on Wednesday that it was most likely present in many other countries where it had not yet been detected.

Tedros warned that countries might be underprepared to fight the variant.

"Surely, we have learned by now that we underestimate this virus at our peril. Even if Omicron does cause less severe disease, the sheer number of cases could once again overwhelm unprepared health systems," he said.

Studies suggested Omnicron can escape existing vaccines or natural induced immunity, increasing the risk of reinfection among the population.

Tedros said countries should start giving booster vaccine doses to the most vulnerable groups like the elderly and the frontline and health workers. Indonesia has been giving booster shots to the medical workers in the past few months and will start delivering the shots to a broader population next year.

An experimental Covid-19 treatment pill, called molnupiravir and being developed by Merck & Co Inc and Ridgeback Biotherapeutics LP, is seen in this undated handout photo released by Merck & Co Inc and obtained by Reuters May 17, 2021.

Thailand's Public Health Minister Anutin Charnvirakul said on Friday the country planned to develop the anti-viral pill molnupiravir to combat Covid-19 infections amid rising infections driven by the Omicron coronavirus variant.

The molnupiravir Covid-19 treatment pill for adult patients at risk of developing severe illness was jointly developed by US-based Merck & Co Inc and Ridgeback Biotherapeutics.

The Southeast Asian nation intends to join other countries in the region also planning to make versions of the drug, including Bangladesh and India.

"For molnupiravir, the Government Pharmaceutical Organization has plans to co-develop (this drug) with the Chulabhorn Research Institute," Mr Anutin told a news conference.

"A committee will research and produce the reactants, which will be additional support for future events," he said.

Separately, Indonesia's health minister Budi Gunadi Sadikin said on Friday the country also planned to develop molnupiravir in partnership with pharmaceutical company PT Amarox Pharma Global starting in April or May.

Indonesia approved molnupiravir for emergency use this week and 400,000 pills have arrived.

Thailand has ordered 50,000 courses of molnupiravir from Merck and was still in discussion with Pfizer for 50,000 courses of its drug Paxlovid.

Natco Pharma gets non-exclusive licence to manufacture, sell anti-COVID pill Molnupiravir
Sunil Matkar Natco Pharma gets non-exclusive licence to manufacture, sell anti-COVID pill Molnupiravir

Drugmaker Natco Pharma Ltd on January 20 announced it has inked an agreement with the Switzerland-based Medicines Patent Pool (MPP) for a non-exclusive license to manufacture and sell anti-COVID pill Molnupiravir.

The deal was signed with MPP as it has taken licence from the US-based Merck Sharp & Dohme Corp (MSD) - which has developed Molnupiravir - for the same, the company said in a statement.

"Natco with this license agreement can manufacture and sell Molnupiravir Capsules 200 mg. for Indian market, which will be sold under brand name MOLNUNAT ® for treatment of COVID-19 infection with Sp02>93% and who have a high risk of progression of the disease including hospitalisation or death," it added.

This agreement allows Natco to expand access to COVID-19 medicines in 105 countries in generic name. Under the licence, the pharmaceutical firm can set its own price for the generic products it produces, paying a royalty on sales to MSD.

"MSD, Ridgeback Biotherapeutics and Emory University will not receive royalties for sales of Molnupiravir under this agreement for as long as COVID-19 remains classified as a Public Health Emergency of International Concern by the World Health Organization," Natco clarified.

Molnupiravir was cleared for emergency use on December 28 by the Drug Controller General of India (DCGI). Earlier, the anti-COVID pill was cleared for restricted use by the UK drug regulator on November 4 and was subsequently approved for emergency use in the United States.

The Indian Council of Medical Research, however, has so far not included Molnupiravir in the clinical management protocol for COVID-19 due to safety concerns. The Union health ministry on January 12 said the drug has risks and warrants caution in use.

According to Merck, which released data on Molnupiravir's trials in October, the drug demonstrated highly promising results in reducing viral load to almost zero in merely five days of treatment duration.

Only firm licensed to manufacture Molnupiravir in Vietnam

21/01/2022    13:10

The Medicine Patent Pool (MPP) on January 20 announced it has granted licenses to manufacture the COVID-19 treatment drug Molnupiravir to 27 pharmaceutical firms worldwide, including Stella Vietnam (Stellapharm).

Only firm licensed to manufacture Molnupiravir in Vietnam

The antiviral drug molnupiravir is given to mild F0 cases for COVID-19 treatment.

A Stella Vietnam representative confirmed the news with tienphong.vn.

Besides Stella Vietnam, the MPP also granted its licenses to 10 companies in India, five in China, three in the Republic of Korea, two each in South Africa and Bangladesh, and one each in Egypt /Jordan, Kenya, Pakistan and Indonesia.

The MPP website says MPP has signed agreements with 27 generic manufacturing companies for the manufacturing of the oral COVID-19 antiviral medication molnupiravir and supply in 105 low- and-middle-income countries (LMICs).

The sublicence agreements are the result of the voluntary licensing agreement signed by MPP and MSD, a trade name of Merck & Co., Inc, Kenilworth, NJ, USA, in October 2021 to facilitate affordable global access for Molnupiravir, that MSD is developing in partnership with Ridgeback Biotherapeutics.

“We are encouraged by the large number of new and existing partners that have moved quickly to secure a sublicence for molnupiravir through MPP,” said Charles Gore, MPP Executive Director. “This is a critical step toward ensuring global access to an urgently needed COVID-19 treatment and we are confident that, as manufacturers are working closely with regulatory authorities, the anticipated treatments will be rapidly available in LMICs.”

Molnupiravir, the antiviral drug, has been approved for treating COVID-19 patients with mild to moderate symptoms. Studies show the medication can help reduce the numbers of hospitalisations and deaths in COVID-19 patients.

In Vietnam, the Ministry of Health has approved a pilot programme using Molnupiravir to treat COVID-19 cases at home. Preliminary reports show almost 100% of patients taking the drug had a low viral load, a very low rate of severe disease, and no deaths.

JAKARTA, Indonesia (AP) — Indonesia kicked off a COVID-19 booster campaign for the general public on Wednesday, prioritizing third shots for the elderly and people with compromised immune systems.

The government hopes to provide 21 million booster shots in January to people who received their second jabs at least six months ago. Some 117 million people in Indonesia have already received two doses of the vaccine.

“This effort is important to increase the immunity of society, considering the COVID-19 virus keeps mutating,” President Joko Widodo said Tuesday.

Indonesia has reported more than 4.2 million cases and more than 144,000 deaths since the pandemic began. During the peak of the last surge in July, the country recorded 56,757 cases per day as hospitals were overwhelmed by sick patients and ran out of beds and oxygen supplies.

The country provided the Moderna vaccine as a booster for healthcare workers starting in July last year.

Indonesia’s Food and Drug Authority on Monday approved emergency-use authorization for the Sinovac, Pfizer, AstraZeneca, Moderna and Zifivax vaccines, all of which will be used as booster shots. Several other vaccines are also being examined for emergency-use authorization.

Indonesia’s Health Minister Budi Gunadi Sadikin said the government would consider the availability of vaccines in determining which boosters people will receive and that the boosters might be different from what people received in their first two doses.

People who received the Sinovac vaccine as their primary doses will receive half a dose of the Pfizer vaccine or half a dose of the AstraZeneca vaccine as a booster, while those who received AstraZeneca’s vaccine as their primary doses will receive half a dose of the Moderna vaccine as a booster.

Covid antiviral pill launched at Rs 1,399 for full 5-day course

MUMBAI: Molnupiravir, the antiviral drug which recently got emergency use approval for treatment of mild to moderate Covid-19 in India, was launched on Monday at Rs 1,399 for the full five-day course, making it one of the cheapest antiviral coronavirus therapies during the pandemic.

Over a dozen companies, including Hetero, Sun Pharma, Natco and Dr Reddy’s are in the process of rolling out the oral therapy developed by Merck and and its partner, Ridgeback Biotherapeutics, at prices ranging between Rs 1,500 and Rs 2,500 for the complete treatment, sources told TOI. The antiviral pill is being considered a potential gamechanger globally in Covid treatment as remdesivir used till now is administered intravenously, and in hospital settings.

Mankind Pharma, which has tied up with BDR Pharmaceuticals. introduced its antiviral Molulife (200 mg) in Delhi and certain other parts of the country on Monday, a company official said. Further, Sun Pharma has priced the antiviral at around Rs 1,500 for the full course, and is making its ‘Molxvir’ available in markets where there are more cases, and gradually across India.

The recommended dose of molnupiravir is 800 mg twice a day for five days. Given the stiff competition among the market players, the treatment cost will remain competitive and affordable for patients, experts said. Molnupiravir, is also the first oral anti-Covid pill approved by UK’s drug regulator, while recently the US Food and Drug Administration cleared it for the treatment of mild-to-moderate Covid-19 in adults, and for those who are at a high risk for progression to severe disease, including hospitalisation or death.

Post the USFDA nod, Drugs Controller General of India granted permission for restricted use of molnupiravir for treatment of adult patients with SpO2 93%, and who have high risk of progression of the disease.

Earlier in June, MSD signed licensing agreements with five generic companies including Sun Pharma, Cipla, Torrent, Emcure and Dr Reddy’s to manufacture and supply molnupiravir in India, and to over 100 low and middle-income countries.

So, what are the health foods we should eat then?

Just my opinion -

For a lean body mass: eat small meals, with plenty of water and mineral salts (pink Himalayan salt).

You are like a hydro electric plant. You get the hydro part from water and you get the electrolytes from you mineral salts.

I use the general concept of the following:

-Fill with fat

-Prioritize proteins

-Control carbohydrates.

Fat goes right to the liver and is converted to ketone bodies which is your brain most useful and favorite food.

Fat turns into cholesterol, cholesterol turns into steroids, steroids turn into hormones and hormones run your body.

Every cell is made of a lipo - protein membrane not a carbohydrate membrane and never MSG or food coloring, etc.

Large meals and meals with primarily carbohydrates use insulin to feed the cells.

Small meals uses glucagon to feed the cells. Glucagon will eat cancer cells and insulin will feed cancer cells.

I eat a "modified Atkins diet" I love carbohydrates; rice, beans, corn, potatoes, bread, pasta, fruits and vegetables - I just don't eat them much, if at all.

I stay away from most everything human processed. If you have to "read the label" then I would put it back.

I can't even pronounce the words let alone know what they mean.

All fruit coming from out of the country is radiated to kill bugs. How much radiated food should we eat?

I am always tight for time so I use the crock pot a lot. Meaty, fattie, watery with some salt, pepper and very little

carbohydrates.

Think back prior to the invention of fast foods. You know the olden days.

Hope it helps...

Thanks bigwave. Did that work for people with a diabetes, too? Can diabetes people be allowed to eat foods that contain high fat, protein, salt, carbohydrates? And, can diabetes people be allowed to eat fruits ( that taste sweet,  like watermelon, banana, pumpkin, pineapple, apple ), rice, sweet potatoes, etc? 4th vaccine dose may be needed sooner than expected to fight Omicron variant: Pfizer CEO

However, two doses of Pfizer likely still offers protection against getting seriously ill from Covid infection.

4-dose-omicron.jpg

Shortly after recommending that a booster third dose of the Pfizer-BioNTech vaccine should be taken to neutralise the Omicron variant, the Pfizer CEO said that a fourth dose may also be needed "sooner than expected".

Speaking to CNBC, Pfizer CEO Albert Bourla said that his company will look at real-world data to determine how well a third dose will fare against the new variant, and predicted that a fourth dose administered in a timely manner might be needed to effectively combat it.

Fourth dose may be needed sooner than 12 months after third

Bourla said: "I think when we see real world data will determine if the Omicron is well covered by the third dose and for how long....and I think we will need the fourth dose. I’ve said that multiple times."

He had earlier projected that with the previous variants, the fourth dose would be required 12 months after the third dose.

"With Omicron, we need to wait and see because we have very little information. We may need it faster," he added.

However, the Pfizer CEO stressed that "the most important thing" for now is for people to get their third doses in time for winter.

More protection is needed in the winter months, as more people gather indoors to escape from the cold, CNBC reported.

In a joint statement released by Pfizer and BioNTech on Wednesday (Dec. 8), preliminary results found that a three-dose course of their vaccine would be able to effectively neutralise the Omicron variant.

"If there is a need for something different better, we will be able to have in months," said Bourla.

Pfizer and BioNTech said on Wednesday that they are already working on an Omicron-specific vaccine, and may be able to deliver them by March 2022.

Boosters for children & teenagers

Bourla was also asked when teenagers, most of whom had recently completed both doses of the vaccine, should get a third booster shot as well.

In response, he said:

"This is a good question and in light of Omicron that we need to move faster on this. I think third boost, third doses was a very good policy and they need to be accelerated. This is what the Omicron results are telling them. You need the third boost."

Singapore Health Ministry probing erroneous 4th Covid-19 vaccine dose given to 103-year-old nursing home resident

The Singapore Ministry of Health has reminded all mobile vaccination teams to perform independent identity verification and authentication before administering any Covid-19 vaccination

SINGAPORE, Feb 5 — The Ministry of Health (MOH) is looking into the death of a 103-year-old nursing home resident who, in a case of “mistaken identity”, was wrongly administered with a fourth dose of a Covid-19 vaccine by a mobile vaccination team.

Preliminary findings by the ministry showed that the vaccine was erroneously administered due to “possible irregularities in vaccination procedures and poor communication between the nursing home and the medical service provider handling the vaccination”, MOH said in a statement on Friday evening.

An autopsy ordered by a coroner found that the main cause of the woman’s death was pneumonia, with other contributing factors being cerebral infarction (or stroke) and coronary artery disease, which are natural disease processes common in seniors, MOH added.

“The coroner has not determined whether these causes of death were linked to the vaccination.”

What happened

In its statement, MOH said that the woman, who is a resident of Econ Healthcare  —  Chai Chee Nursing Home, had already received three doses of a Covid-19 vaccine.

She was wrongly given a fourth shot on December 13 last year by a mobile team from PanCare Medical Clinic.

In response to TODAY’s queries, Econ Healthcare said that the mistake was discovered about five minutes after the vaccine was administered.

The teams from Econ Healthcare and PanCare Medical Clinic attended to the woman and extended the post-vaccination observation time.

“The resident had no adverse reaction during that time,” Econ Healthcare said.

A few days later on December 16, she was admitted to Changi General Hospital for pneumonia and hyponatremia, a condition where sodium levels in the blood are lower than normal, usually due to too much water in the body. The woman was later diagnosed to have suffered a stroke as well.

She died on January 10 this year.

“This is the first case of mistaken identity leading to erroneous vaccination by a mobile vaccination team in over 152,000 vaccinations to date,” MOH said.

The ministry said that it takes a serious view of this incident and is carrying out a thorough investigation. It expects the investigations to conclude this month.

Delayed announcement

MOH also said that it had planned to announce this incident in December last year, but had delayed doing so because the woman’s family had asked to withhold details, which could have led to the identification of the deceased.

“We have since consulted the family further and are releasing the information to provide clarity on the incident.”

Econ Healthcare Group and PanCare Medical Clinic have funded the the woman’s hospital bill “as a goodwill gesture”, the ministry said.

Econ Healthcare has also been in contact with the woman’s family to render support to them.

Econ Healthcare and PanCare Medical Clinic have reviewed their processes to prevent a recurrence, MOH said.

The Agency for Integrated Care, which facilitates vaccinations in nursing homes, has reminded all nursing homes to ensure proper communications with the mobile vaccination teams when vaccination takes place.

“MOH has also reminded all mobile vaccination teams to perform independent identity verification and authentication before administering any vaccination,” the ministry added. — TODAY

COVID booster shots may be needed every year: Ong Ye Kung

Health Minister Ong Ye Kung addresses Parliament on 10 January 2022

SINGAPORE — Just as individuals take a yearly flu shot, Singaporeans may also need to take a yearly booster shot as Singapore continues to transit into the endemic phase of the pandemic, said Health Minister Ong Ye Kung in Parliament on Monday (10 January).

Responding to a query from Tampines Member of Parliament Cheng Li Hui on whether more vaccination doses are needed after the booster jab, Ong said that it was "too early to tell". He noted that Israel is currently the only country that has authorised a fourth dose for non-immunocompromised individuals.

Ong pointed to an "endemic, infectious disease" like influenza, which mutates frequently. "So people receive vaccinations every year to protect themselves against it without many problems, or the need for disruptive border closures and social restrictions each time there’s an influenza infection wave.

"It is a possible future scenario when we live with COVID-19 as an endemic disease."

The minister added that the Ministry of Health and the Expert Committee on COVID-19 Vaccination (EC19V) will continue to monitor local and international data on the durability of protection from vaccine boosters and the evolution of the virus to assess the need for further vaccinations.

Separately, Senior Minister of State Janil Puthucheary told the House that 15,540 children aged below 12 were infected with COVID last year. A total of 3,145 were sent to hospital or a COVID-19 facility but have all been discharged.

Since mid-October last year, Singapore has seen several serious cases in children, with some requiring intensive care. "Although fewer children have been seriously ill with COVID-19 compared with adults, there is still a risk of them becoming seriously ill or developing severe COVID-19 complications such as multi-system inflammatory syndrome in children (MIS-C)," said Dr Puthucheary.

Health authorities are also reviewing if the Sinovac-Coronavac vaccine can be extended to children aged five to 11, who are medically ineligible to take the Pfizer-BioNTech Comirnaty COVID-19 vaccine.

The review will be based on further studies on the safety and effectiveness of the Sinovac vaccine.

At present, only the Pfizer vaccine is authorised for use in children below 18 years. Special exemptions are made for those aged 12 to 17 who are not medically eligible for it to receive a Sinovac-Coronavac vaccine under a dedicated public health program.

What kinds of foods that diabetes people should eat?

Not Bigwave, speaking from a diabetic point of view. High fat foods, (avocados, some nuts, bacon, eggs) medium protein  and low carb to none. Salads and most vegetables are good. They have carbs but they also have high fiber. Pink salt or sea salt is good. Even on a Keto diet portion control is important. I don't subscribe to the "eat every 2 hours" thought because if your blood glucose doesn't go down you end up with a high glucose level all the time. Keto works for me. It doesn't work for everyone.

What foods are allowed on a ketogenic diet?

Foods that are generally allowed include high-fat meats, fish, oils, nuts, high-fat dairy such as cheese, and low-carb vegetables such as leafy greens.

Unsurprisingly, reducing carb levels means cutting out bread, pasta, rice, and most conventional baked goods. However, achieving such low levels of carbs also means skipping legumes, root vegetables, most fruits and starchy veggies, such as potatoes.

Is it safe?

This eating plan certainly contradicts most people’s understanding of a healthy, balanced diet which typically promotes the consumption of protein, fat and carbohydrates. From an evolutionary perspective, ketosis is a normal adaptive response which enabled humans to withstand periods of famine throughout history. Today, this natural physiological mechanism has been exploited by a number of low-carb diet regimes. Following such a diet means you will be replacing carbs with foods rich in fat and protein, and if followed over an extended period of time this may have unfavourable consequences for some individuals. An intake of high fat foods is likely to increase your saturated fat intake which current UK government guidelines recommend that we limit to 30g for men and 20g for women. High levels of dietary protein are thought to be an issue if you have an underlying kidney condition. However, most ketogenic diets supply moderate rather than high levels of protein.

Who should be more careful about following a ketogenic diet?

Diabetics, especially, type 1 diabetics are at risk of complications if they attempt to follow a ketogenic diet. For this reason diabetics and anyone with a blood sugar management issue should discuss the potential implications with their GP and healthcare team before embarking on such a regime. Similarly anyone with kidney disease or a family history of such should consult their GP.
