Can't signup for VIP

Probably a dumb question, but I've been trying to signup for the VIP membership.  What is an "Existing VIP Certificate Number/Name or OSI Username:"?

Just enter your name (not user ID). Since you are not a VIP member now there is no Certificate number.

I tried to sign up for the special lifetime VIP yesterday. It kept saying transaction declined. I tried to sign up four times and again transaction was declined. I then pulled up my credit card account to see if there was a problem there and saw that my account had been charged four times for the lifetime VIP, a total of $770.00.I was also charged for two regular VIP memberships, at a total of $99.98.  I have sent an  email to VIP@hotmail.com but I am asking help anyway I can to have this matter resolved.as soon as possible. I need the money refunded to my account. Thank you for your help.

Have no idea what might have happened but sending an email to VIP@hotmail.com is not going to help.
